Liability Waiver for Volunteer/Participant as a volunteer of Illinois Animal Rescue, Inc., I recognize and acknowledge that there are certain risks of physical injury and I agree to assume the full risk of any injuries, including death, damages or loss which I may sustain as a result of volunteering in any and all activities connected with or associated with Illinois Animal Rescue, Inc.I do hereby fully release and discharge Illinois Animal Rescue, Inc, their officers, agents, servants, and employees from any and all claims from injuries, including death, damages or loss which may accrue to me on account of my volunteering with Illinois Animal Rescue, Inc.I further indemnify and hold harmless and defend Illinois Animal Rescue, Inc, their officers, agents, servants, and employees from any and all claims resulting from injuries , including death, damages or loss sustained by me and arising out of, connected with, or in any way associates with my volunteering with Illinois Animal Rescue, Inc. By submitting the foster application the applicant agrees to waiver any and all liability claims against IAR; the foster home will have space to seperate/confine foster animals from any resident pets; foster home will contact IAR prior to taking a foster animal to a veterinarian; foster home will incur no cost to IAR on behalf of the foster animal without first obtaining written approval by IAR designated representative; foster homes will keep detailed records of any medical treatments and care given to the foster animal; Foster home will abide by all IAR's medical procedures, ensure all medical schedules are followed and keep proper documentation; Foster homes must have their pets current on IAR and vet recommended vaccinations and spayed or neutered; If the foster animal becomes lost or runs away, the foster home must immediatly contact IAR the local police and animal control; foster homed animals must have proper identification on at all times; Foster home shall agree to give the animals in their care, sufficient quantity of good quality qholesome food and fresh water, adequate shelter and protection from the wather or injury, veterinary care when needed and the appropriate socialization needed, as set forth by the Illinois Humane Care for Animals Act and IAR; IAR has the right to inspect the foster home at any time or to request the return of and foster animal at any time which will be released to IAR within a 24 hour period; The foster home is responsible for the annual $25 Foster Home licensing fee, as set forth by the IL Department of Agriculture; Foster animals will be assigned to foster homes at the disrection of IAR; Only a designated representative has the authority to accept animals into the organizaion or its foster homes; this application and all paperwork regarding fostering IAR's animals as well as the animals themselves are the sole property of Illinois Animal Rescue, Inc. |
